ICMR study reconfirms COVID-19 spreads faster among close contacts

New Delhi: The novel coronavirus has a higher rate of transmission among close contacts and thus, public health measures such as physical distancing, personal hygiene and infection control are necessary to prevent its spread, the Indian Council of Medical Research (ICMR) has reconfirmed.
Sharing the findings of the first cluster of SARS-CoV-2 infection among Italian tourists in a study, the ICMR also said testing of close contacts identified infection in presymptomatic and asymptomatic cases, and stressed that the strategy to trace and test close contacts is crucial for early identification and isolation of positive patients to prevent community transmission.
A detailed investigation of the cluster of SARS-CoV-2 infection among 16 Italian tourists and an Indian was conducted in March-April, an ICMR study published online in the Indian Journal of Medical Research (IJMR) said.
A group of 23 Italian tourists reached New Delhi on February 21 and along with three Indians, visited several tourist places in Rajasthan.
One of the members from the group, a 69-year-old Italian male (index case), was hospitalised at the Sawai Man Singh (SMS) Medical College in Jaipur, with symptoms of fever, cough and difficulty in breathing on February 29, and tested positive for the infection.

His 70-year-old wife, who did not have any symptoms, also tested positive for COVID-19 and was isolated, along with him.
The remaining 24 members of the group (21 Italians, three Indians) returned to Delhi on March 2 travelling in the same train compartment and were quarantined.
All them were initially asymptomatic. Their throat and nasal swabs were collected on March 3 and 15 of them (14 Italians and an Indian) tested positive and were isolated.
So by March 3, 17 of the 26 were COVID-19 positive with an attack rate of 65.4 per cent.
Of these 17 patients, nine were symptomatic, while eight did not show any symptoms. Of the nine who developed symptoms, six had a mild fever, one was severely and two were critically ill.
“The median duration between the day of confirmation for COVID-19 and RT-PCR negativity was 18 days (range: 12-23 days). Two patients died with a case fatality of 11.8 per cent.
“This study reconfirms higher rates of transmission among close contacts and therefore, public health measures such as physical distancing, personal hygiene and infection control are necessary to prevent transmission,” the apex health research body said.
“Our study cluster showed a higher attack rate than that reported in existing literature such as in Diamond Princess Cruise ship (19.2 pc) and Grand Princess Cruise ship (16.6 pc),” which the ICMR said was may be due to the closed environment, high and persistent exposure to the index case during their travel (average of six hours daily for eight days).
Except for the index case, all other cases were asymptomatic at the time of testing and nearly half of the positive cases remained asymptomatic throughout the illness, the study highlighted.
Proactive COVID-19 testing of close contacts led to the identification and isolation of the asymptomatic and pre-symptomatic cases, thus preventing further transmission. Older patients with chronic comorbidities progressed to severe or critical illness, as reported in other studies.
The proportion of symptomatic patients progressing to severe or critical illness in the cluster was high (3/9, 33.3 pc). The case fatality ratio (CFR) in the affected cluster was 11.5 per cent.
“The possible reasons for the CFR being on the higher side could be the higher median age group of the patients and presence of comorbidities,” the study said.

ICMR releases revised strategy for Covid-19 testing in India




The Indian Council of Medical Research (ICMR) on Monday released revised strategy for Covid-19 testing.
In the revised ICMR guidelines:
All symptomatic individuals with history of international travel in the last 14 days to be tested.
It also says that all symptomatic contacts of laboratory confirmed cases, healthcare workers at the front-line of battle against Covid-19 and patients of Severe Acute Respiratory Infection or SARI to be tested.
Asymptomatic direct and high-risk contacts of the people who have been tested positive with the virus must be tested between day 5 and day 10 of coming into contact.
All symptomatic ILI within hotspots/containment zones and hospitalised patients who develop ILI symptoms and symptomatic among returnees and migrants within 7 days of illness must be tested.
No emergency procedure (including deliveries) should be delayed for lack of test.
Also, All the above testing is recommended by the real-time RT-PCR test only.

ICMR teams up with Bharat Biotech Limited to develop Covid-19 vaccine


Indian Council of Medical Research (ICMR) has partnered with Bharat Biotech International Limited (BBIL) to develop the first ‘Make-in-India’ vaccine for Covid-19.
The vaccine will be developed using the virus strain isolated at the ICMR’s National Institute of Virology (NIV), Pune.
Dr. Raman Gangakhedkar, ICMR’s head of epidemiology and communicable diseases said, “ICMR has shared one of the 11 virus strains that it managed to culture with Bharat Biotech. The industry partnership is to develop the vaccine candidate. ICMR has transferred the technology to them,”
Also, the health research body in a statement said, “Work on vaccine development has been initiated between the two partners. The ICMR-NIV will provide continuous support to BBIL for vaccine development. ICMR and BBIL will seek fast-track approvals to expedite vaccine development, subsequent animal studies and clinical evaluation of the candidate vaccine”.

ICMR asks states to use rapid antibody tests for surveillance in COVID-19 hotspots


New Delhi: The Indian Council of Medical Research (ICMR) has written to all states asking them to use rapid antibody tests for surveillance in COVID-19 hotspots.
It underlined that the old standard frontline test for novel coronavirus diagnosis is a real-time PCR (Polymerase Chain Reaction) based molecular test, which is aimed at early virus detection.
In a letter to all chief secretaries and principal secretaries (health) of states, ICMR Director General Dr Balram Bharagava listed the protocol for using the rapid antibody test’ in hotspot area for epidemiological studies and surveillance.
India received five lakh rapid antibody testing kits from China on Thursday and these, health ministry officials, said are being distributed to states for districts with high burden of infection.
Chief of Epidemiology and Communicable Diseases at ICMR Dr Raman R Gangakhedkar on Thursday had said that the rapid antibody test kits procured are not meant for early diagnosis, but “only for surveillance and trend checking”.

“The National Task Force at the ICMR has carefully reviewed the data evolving from various countries on use of such kits. Based on available evidences, the testing strategy for COVID-19 has been revised further,” Bhargava said.
Stating it was critical to understand few key facts while using the rapid antibody tests, Bhargava said gold standard frontline test for COVID-19 diagnosis is real-time PCR based molecular test, which is aimed at early virus detection and the rapid antibody test cannot replace the frontline test.
“The rapid antibody test is a supplementary tool to assess the prevalence of the diseases within a specific area/perimeter. It will only be of utility after a minimum of seven days of onset of symptoms.
“Data about these rapid tests is emerging and understanding of their utility for diagnosis is still evolving. The rapid test kits are useful for epidemiological studies and surveillance purposes,” he said.
The test has to be done under strict medical supervision.
In case, the state does not have a hotspot, these tests may be used for: any hotspot which may emerge in future or as a surveillance tool for epidemiological purpose in such areas where cases have not emerged so far, the ICMR said.
Before the rapid test, it should be registered in the COVI19cc.nic.in/ICMR and data related to the test should be reported on the same, he said.
